Apex Human Services, LLC, provides healthcare in Yeadon, Pennsylvania, that aims toward professional and wholesome care. Our RNs, LPNs, HHAs, and therapists are highly experienced and well-trained in the provision of quality skilled care in your home. We provide shift care to pediatric patients, adults with chronic medical conditions on Trach and Vent requiring around the clock nursing, home visits to geriatric patients, and HHA services. We accept Medicare, Medicaid, Private Insurance, OLTL Waivers, ODP Waiver Services.

Our emergency plan includes back-up nursing to ensure that you and your loved ones are never alone.

Home health services:
Skilled Nursing Adult:
If you are 65 and older, and you participate in the Medicare/Medicaid programs, private insurance, or private pay, and you need skilled nursing after hospital care, Apex RNs will make a home visit to your home to provide skilled nursing services.
This may include but not limited to:
- Wound care
- Medication management
- Infusion of IV Therapy
- TPN Therapy
- Diabetes-Blood sugar checks and glucose monitoring
- Patient teaching on the disease process
- Alzheimer’s care
- Dementia
- Parkinson’s disease
- Blood pressure checks
Skilled Nursing Pediatrics:
For children ages 1 and up requiring one-on-one nursing care at home. Ideally, children with feeding tubes, tracheostomies or ventilator dependent. Under the auspicious of the primary care physicians and other specialist physicians, the nurses work on a shift basis to provide skilled nursing services.
Home Health Aides:
Services include but not limited to:

- House Keeping
Behavioral Health:
At Apex, we regard behavioral health as a way of being inclusive. In other words, people experiencing this health condition should not be isolated. They should have a place in our society. Certainly, they have a place at Apex. In addressing Behavioral health issues, our agency designs ways of promoting the well-being of our individuals served by preventing (ex-ante) or intervening (ex-post) in mental illness such as depression or anxiety. Our support in Behavioral health also extends to our goal of preventing or intervening in substance abuse and/or other addictions.
Behavioral Support Services:
This service is available for individuals age 21 and older and includes comprehensive assessment, the development of and revisions to a behavioral support plan that includes strategies to support the individual based upon assessment, and the provision of interventions and training to individuals, staff, and caregivers.
Developmental Disabilities:
Our Health and Human Services Agency implements programs and services to provide comprehensive, quality home health care in the individual’s home. Apex promotes “Person-Centered” practices and ensures the “Everyday Life” principles are maintained. Our agency is approved by the Office of Developmental Program (ODP) to provide a wide range of services for people with special needs. These services are direct services (face-to-face) to assist the individuals in their homes and community settings, improving their skill level necessary to live and participate fully in community life. In improving the capabilities and socialization skills of the individuals served, our staff is trained to ensure the safety, well-being, and the welfare of the individual are promoted. More importantly, our services encompass assistance, support, and guidance to meet the individuals’ habilitative outcomes as stipulated in the Individual Support Plan (ISP).
Companion: Companion services are provided to individuals living in their own homes for the limited purposes of providing supervision and assistance that is focused solely on the health and safety of the adult individual with an intellectual disability.
In-Home and Community Support: In this program, our agency provides services that aimed at meeting the individual’s needs and interests. We do so by training qualified staff that helps the individuals in receiving assistance and training that empowers them to live their lives as independent as possible. Staff will also provide services that aimed at strengthening the individual’s ability to care for him/herself in their homes, and in the community.
Home and Community Based services provided by Apex utilizes a person-centered planning approach. Our services allow individuals with intellectual disabilities to live in the comfort of their own home or at the community and maintain their independence with the support of committed caregivers. Individuals are supported and encouraged to make their own choices, develop new abilities, and cultivate their special talents in their homes and communities.
Apex supports include, but are not limited to:
- Learning, maintaining, or enhancing a skill, such as cooking or managing finances
- Learning how to use public transportation
- Cultivation of personal care skills
- Completion of household chores or other daily living skills development
- Customized services and supports designed to respond to the needs of the individual and the primary caregiver
- Shopping and cleaning assistance
- Participation in community activities and recreation
- Financial counseling and money management
- Assistance to medical appointments
- Help and support for individuals to connect with others in their natural surroundings or to integrate into their communities
